New Patient Registration

We are welcoming new registrations!

If you are a EU resident (or have the intention to be resident for at least 6 months) within the Peterborough Unitary Authority boundaries, you are eligible to register at the practice.   

We will need you to fill in a few forms: GMS1 Registration form, New Patient Questionnaire and Record Sharing.  You can collect these from reception in the registration pack, which contains our practice leaflet, or you can download the forms below:

We will also want to see the following documents:

  • Government issued Photo ID document (e.g. driving licence or Passport)
  • A utility bill showing your address (as evidence of residence in the area) 

If you are not entitled to NHS care you can still register with the practice as a Private Patient.

The surgery catchment area is very wide and is demonstrated in the image below. We will offer registration to everyone with
in the blue area however only people within the Red highlighted area will be eligible for a home visit.
